Ontario Vital Certificates

Birth, marriage and death certificates are official documents issued by ServiceOntario, through the Office of the Registrar General, for events registered in Ontario. They are commonly required for passports, a Social Insurance Number, a driver’s license, estate and pension matters, insurance claims, name changes and many other everyday processes.

Many people find the request process unclear, especially when ordering on behalf of a child, a relative, an estate or as an authorized representative.

Our role is to help you prepare and submit your request correctly, with your authorization, so the information reaches ServiceOntario complete and error-free.

Which Certificate Do You Need?

Choose a certificate to start your assisted request. Each request has a fixed total price of CAD $99, with the government fee and taxes already included.

Birth Certificate

For a birth registered in Ontario. Requested by the person named on the record, or by a parent named on the birth registration for their minor child.

Marriage Certificate

For a marriage registered in Ontario. Requested by either spouse named on the record, or an authorized representative acting on their behalf.

Death Certificate

For a death registered in Ontario. Requested by an eligible relative, the estate, or an authorized representative acting on their behalf.

Who Can Request a Certificate?

ServiceOntario allows a certificate to be requested by:

  • The person named on a birth record, if of age
  • A parent named on the birth registration, requesting the certificate for their minor child
  • A spouse named on a marriage record, if of age
  • A relative, the estate, or the executor of the person named on a death record
  • An authorised representative acting with the written authorization of an eligible applicant

As a matter of our own service policy, our assistance service is currently available only to residents of Canadian provinces other than Quebec. Eligibility to receive the certificate is determined exclusively by ServiceOntario.
